I hereby grant GSK plc, its affiliate companies and any licensees, successors or assignees, the right to use, edit and distribute the above-described images and likeness (“Images”) and videotaped recordings along with any other rights in the images and videotaped recordings and any other reproductions, adaptations or derivative works of the images and videotaped recordings:

internal and external to GSK, and worldwide

I understand that the images and videotaped recordings may be retained and used by GSK, its licensees, affiliates, successors and assignees, and for any and all purposes.

I will not hold GSK, or its licensees, affiliates, successors, or assignees, liable for any use or distribution of the images and videotaped recordings, and I do hereby release GSK, and its licensees, affiliates, successors, and assignees from any and all claims for damages for libel, slander, invasion of privacy, or any other claim related to use, reproduction, adaptation or distribution of the Images and videotaped recordings.

This Release Form will apply to any minor child of the Participant who participates hereunder.

In granting this consent, I understand and agree that no consideration is owed to me by GSK or any other party. By signing below, I agree to the above use and disclosure of my personal information and represent and warrant that the information I have provided is true and accurate and that I am 18 years of age or older.
