To participate in IMPD North District Bike Unit Youth Mountain Bike Project

I, the undersigned

along with my heirs, executors and administrators, hereby release and discharge the City of Indianapolis, its departments, officers, agents, seNants, and employees (hereinafter "City") from any and all claims, demands, or actions for personal injury, property damage, or any other damage or loss, whether caused by the negligence of Indianapolis Department of Parks and Recreation employees or other City employees, Indianapolis Metropolitan Police Department officers, or other individuals or entities, that may arise out my participation in the IMPD North District Bike Unit Youth Mountain Bike Project, which may include, but is not limited to, instruction and participation in mountain biking activities (the "Activity").

I understand, acknowledge, and voluntarily assume full responsibility for participating in this Activity, which Activity may expose me to dangers of injury from known and from unanticipated risks, and whether due to negligence or otherwise. Such risks include physical injuries which may occur during the course of my participation in the Activity.

I specifically waive any and all claims against the City arising out of my participation in the Activity and agree to indemnify, and save harmless the City, its officers, agents, servants, and employees from every liability, claim, demand, right of action, judgment, loss, cost, damage, or expense (including reasonable attorney's fees) for every injury to, or death of, any person or persons (including agents of the City) or instances of damage to property (including property of the City) out of which injury, death, or damage arises or is in any way connected with the Activity.

Waivers are valid through the end of the current calendar year regardless of when the annual waiver was submitted or renewed. Each will expire on the subsequent 12/31 and must be renewed thereafter. J declare that I have completely read and understand this release and that I voluntarily and knowingly agree to its terms.
